Palmetto Health-USC Medical Group Core Values?All team members are expected to be knowledgeable and compliant with the Palmetto Health-USC Medical Group's core values of Service, Excellence, Professionalism, Partnership, Integrity, Accountability, and Transparency.Position Summary:?To provide psychosocial support services to HIV-infected young adults, including risk reduction, basic HIV education, and medication/medical adherence for the Transitioning, Retaining, Orienting Young Adults (TROY) program of the Ryan White Program. Provide outreach and direct services such as client orientation sessions, support group facilitation, and one-to-one adherence counseling.?Duties:?Meets with young adult patients who are newly diagnosed with HIV to provide peer psychosocial support and to encourage compliance with medical appointments and medication adherence. Makes home and/or hospital visits as necessary. Develops a rapport with patients through the use of listening skills and ease of accessibility.?Provides linkage to adult care at the Ryan White Program of the USC Immunology Center for patients transitioning from adolescent care at the USC Pediatric Young Adult Transition Clinic (YATC). Works in collaboration with the staff at the pediatric clinic to ensure a smooth transition.?Assists medical staff in planning and coordinating various programs and activities to meet the psychosocial and health needs of patients and their families, including use of support groups and orientation sessions. Identifies existing community resources and refers patients to medical case management, resources, and programs as needed.?Attempts to locate TROY (Transition Retaining and Orienting Young Adults) patients who have been lost to care and to re-establish medical care.?Documents contacts with patients and services in the Provide Enterprise database and electronic medical record.? Maintains accurate data and provide written reports in a timely manner to meet grant requirements.??Participates in agency staff meetings and serve on agency and/or community committees as assigned. Exchanges relevant client information with?medical?case manager and other clinic staff including medical providers.?Performs other duties as assigned.?
Minimum RequirementsHigh school or GED and two (2) years' related experience in health or social services related area.? Prefer experience working with low-income patients/clients aged 18-24.?Licenses/Certifications/RegistrationsN/A?Driver's License Required?Yes
Knowledge/Skills/AbilitiesTime management is essential, as the job requires meeting with clients and healthcare professionals but also entering and managing a great deal of patient information in the Ryan White electronic database Provide Enterprise and on specific forms. Judgment and critical thinking are needed to structure the time for these duties. Ethical and professional boundaries must be maintained while also engaging clients in a welcoming manner which encourages clients to establish trust based on the knowledge and caring approach provided by the Peer Patient Advocate.??
