Physician Office LPN for Chronic Care Management- Palmetto Adult
The Physician Office LPN assists the physician in assessing the
needs of patients based on data collected through history,
observation, physical exam, telephone triage and analysis of
diagnostic data; takes vital signs, history and asks patient for
chief complaints; documents all relevant information and treatment
on the patient's chart; prepares medication and gives the
injections as ordered by the physician; assists the physician with
treatments and procedures; performs other duties necessary in
providing high quality patient care; possesses skills and abilities
required in a multifaceted environment in addition to those
relating to nursing.
Oversee the office-wide Chronic Care Management (CCM) program to
- Enroll qualified patients into CCM, either by meeting with them
at the time of their appointment or by phone.
- Coordinate development of detailed care plans for enrolled
patients in accordance to all CCM guidelines across all
- Ensure that CCM service elements for all enrolled patients are
completed and billed monthly.
- On-Call will be required after hours, weekends, and/or
80 hours bi-weekly as scheduled
High school diploma or GED equivalent and a graduate of an
accredited licensed practical nursing program. SC nursing license
required. BLS required.
One-year experience in a physician's office or other clinical
setting required. Prefer experience with Chronic Care Management
(CCM) care plans and billing.
