Job Description:
At U.S. Renal Care, we are committed to enhancing the lives of
our patients by providing exceptional dialysis care in a
comfortable home setting. We are seeking a dedicated and skilled
Dialysis Registered Nurse specializing in Home Hemodialysis (Hemo)
and Peritoneal Dialysis (PD) to join our team. This role involves
managing the patient's overall care plan, coordinating with other
healthcare professionals, and ensuring treatments are administered
effectively and compassionately. Our nurses are crucial to helping
patients manage their condition and enhance their quality of life.
We require individuals who are passionate about healthcare and have
a special interest in nephrology, patient education, and holistic
care. Perform patient assessments and create detailed nursing care
plans. Administer and manage home hemodialysis and peritoneal
dialysis treatments. Educate patients and their families on disease
management and prevention. Monitor patient responses to treatments
and adjust plans as necessary. Ensure proper documentation of all
patient interactions and treatments. Coordinate care with other
healthcare professionals as part of a multidisciplinary team.
Perform troubleshooting and maintenance of home dialysis machines
and equipment. Proficient in providing hemodialysis and peritoneal
dialysis. Strong interpersonal and education skills for patient and
family communication. Ability to work independently and manage time
effectively. Strong clinical judgment and decision-making skills.

401k Current Registered Nurse (RN) license in the state of
practice. Certified Nephrology Nurse (CNN) or Certified Dialysis
Nurse (CDN) preferred.
