Job Description:
Agape Care Group is seeking a compassionate and dedicated Nurse
Practitioner to join our team of hospice and palliative care
professionals. In this role, you will provide advanced nursing care
designed to relieve suffering and improve the quality of life for
patients with serious, chronic, or life-threatening conditions. Our
Nurse Practitioners play a pivotal role in our mission to deliver
personalized support and dignity to patients during critical times.
At Agape Care Group, you will be part of a supportive team deeply
committed to delivering exceptional care and comfort to both
patients and their families. Conduct comprehensive patient
assessments to diagnose and treat complex conditions. Develop and
implement patient management plans, record progress notes, and
assist in provision of continuity of care. Prescribe medication and
recommend dietary and lifestyle changes to enhance the quality of
life for patients. Collaborate with interdisciplinary healthcare
teams to formulate holistic, effective care plans. Educate patients
and their families on diagnosis, end-of-life care, and other
relevant health issues. Supervise and mentor nursing staff to
promote best practices in hospice and palliative care. Strong
clinical skills with a focus on geriatric and hospice care.
Excellent communication and interpersonal skills. Ability to work
collaboratively in an interdisciplinary team. Compassionate care
with a strong emphasis on ethics and patient privacy. Medical

Current Nurse Practitioner license. Certification in hospice and
palliative care is preferred but not required.
