The Case Manager assumes clinical management and outcomes of high cost/high risk patients by working with health care providers, patients and family to develop cost effective plans of care. Assists with comprehensive care planning, implementation of care plans, communication and evaluation of progress toward goals. Ensures appropriate level of patient/family health knowledge for discharge.
